BMDC Support Planning Pilot
A key part of the Self Directed process within Personalisation is the Support Planning stage.
Prior to this, customers will have been through the assessment process using the Self Directed Questionnaire (SDQ), a document which aims to establish the person’s care outcomes and which they can complete themselves or with the help and support of a social worker or anyone else within their support network.
The results of the SDQ go through to the Resource Allocation System (RAS) which allocates an indicative budget – the amount of money that person is likely to need to meet their care outcomes.
The Support Planning stage then establishes how the person will meet their care outcomes using their budget – essentially, how do you effectively use this money to do the things you want to do and which meet your care outcomes?
This then leads to the Brokerage stage, finding the right services and putting them in place. Support Planning and Brokerage are not necessarily two separate stages, the lines are very much blurred. In the process of putting together a good support plan it might be that much of the brokerage stage is also covered.
Bradford Council recognises that the Support Planning stage may not be best served by in-house services and is seeking interested parties with whom it might work on a pilot project.

Bradford MDC are developing a Directory of Services which will help them to broker services for customers going through self-directed support.
At the moment the Directory is for internal use only but the intention is that it will be made widely available. We would imagine that many of your services are free to access but for those which are not, and particularly those which people may wish to buy using their Individual Account, we recommend highlighting them in some way on the form so that social care professionals can use the data effectively.
